Consent to the processing of personal data

I hereby voluntarily grant my consent to INDIVIDUA spol. s r.o., with its registered office at Na Palouku 1321, Nová Hospoda, 251 68 Kamenice, VAT: CZ07707576, as the administrator, to process my personal data provided in this form, namely my name, surname, e-mail address and telephone number. The purpose of the processing is to respond to my request for services and to provide information electronically or by telephone by INDIVIDUA spol. s r.o., including sending commercial communications pursuant to Act No. 480/2004 Coll., on certain information society services, as amended.

Data processing

Personal data will be processed both manually and automatically. I acknowledge that I have the right to request information from the controller regarding the processing of my personal data, including the purpose of the processing, the categories of personal data processed, the recipients of the data, and any automated processing. I have the right to request an explanation, rectification, completion or destruction of my personal data if I discover or believe that the processing is not being carried out properly.

Duration and withdrawal of consent

I grant my consent for a period of 60 months from the date of its granting. I am aware that I can withdraw my consent at any time.
